MySQL DBA Survey

For the summer issue of MySQL Magazine (http://www.mysqlzine.net) I have a writer who has kindly volunteered to conducting a survey of current practices, job responsibilities and salaries. Before the survey is put online it would be very beneficial to get input on questions for the survey.

Sample questions:

* how many servers do you manage

* do you have any mysql certifications

The more input we get from the community at this stage the better the survey will be.  Please take the time to put some thought into this and post at least one possible question!

I would be most interested in seeing a confirmation of the changing trend in smaller organizations to have one person do all or a multitude of tasks.

A question of dedicated resources.
Do you have at least one full-time Data Architect/System Architect?
Do you have at least one full-time DBA?
Do you have at least one full-time System Administrator?
Do you have at least one full-time Developer?
Do you have at least one full-time Quality Assurance/Control person?

Questions could be reworded to how many full-time of these people do you have.

Conversely, do you have a resource that is responsible for DBA, System Administration and other tasks.

And, are you responsible for DBA on more technologies other then MySQL, and which ones.

I would also like to sample what skills people have outside of MySQL, because we see a lot of people with previous backgrounds.

So if you could have something like.
Before working on MySQL, did you work on a different Relational Database product, and what was it.
